I have applied to become a mentor with the Lung Transplant Foundation Mentorship Program. The Lung Transplant Foundation requests your reference on my behalf as part of the qualification process.

The Mentorship Program is an all-volunteer effort that connects those who are new or adjusting to lung transplant with mentors, who are trained and have experienced lung transplant as a recipient or caregiver. This mentorship is a one-on-one confidential relationship for the purpose of support and guidance.

Mentors are trained to provide support and information but do not give medical advice. The primary attributes we look for in a mentor are:

  • They are good listeners;

  • They will maintain patient confidentiality; and

  • They are good role models.

Please complete within 7 days.

The Lung Transplant Foundation is a 501(c)3 non-profit organization.
